MAC Users – Player/Volunteer Form Prints Blank

Mac Users in order to print your Player/Volunteer form you need to first download the latest version of Adobe Reader onto your MAC Computer.
Please go to http://get.adobe.com/reader/ and download the latest Adobe Reader for MAC OS.
After you have installed Adobe Reader on your Computer click on the Printer Icon.
By default MAC OS opens up the PDF application with Preview as the viewer which displays a blank form when you click OK
Blank Form:
What you need to do is Click on the Printer Icon and Select Open with OTHER. Then Click OK.
Select Adobe Reader from the Applications Folder and Click Open
The form will load with all the information on it and will be ready to be printed
